~ Sports Physicals ~

Once your child starts playing sports in school they will be required to get a sports physical when entering the 6th, 8th, 9th and 11th year of school.  Our providers set aside a portion of a day for Sports Physicals.    A notice will go out with the packet of information you receive from the school your child will attend.  In this packet you should find the Sports Physical form. This is a two sided form.  One side is the parent/patient questionnaire.  Please have this side filled out and signed prior to the appointment.  You should call as soon as possible to schedule your child’s Sports Physical.  This visit is not billed to your insurance.  There is a flat fee of $20.00 per child.

If your child gets their Sports Physical on a day other than the one set aside, then the fee is $60.00, or we may bill it to your insurance and you would be responsible for only the co-pay, provided the visit is scheduled as a well child exam, with a sports physical.
