New Patients

While our providers are accepting new patients we do have some exceptions. These include new patients coming to us due to:

· an on-the-job-injury or work comp claim,

· a motor vehicle accident,

· a disability claim,

· are on VA

· or any new patients that are currently taking chronic pain management medication.

If you are currently on any prescription medications and would like to establish care with one of our providers, please call the office and discuss this with a nurse or medical assistant. If you have questions about whether or not you would be accepted as a new patient, please call the office to discuss your care.

If you are interested in seeing one of our providers please contact us either by phone, by email, or stop by in person.

If you are seeking OB care, please call our office to discuss your insurance coverage.

Before your first office visit, please contact your insurance company to find out if they will cover a visit to one of our providers. Payment of co-pays or deductibles are due at time of service. Please arrive at least 15 minutes prior to your appointment time to complete your paperwork. If you would like to get started before your appointment, you can print a copy of the registration form to complete and bring to your appointment with you.
