Physical Exams  

 

Also known as an Annual, a Comprehensive Medical Review, or a Well Adult Exam.  At this exam you will be asked to complete a form called a Review of Systems.  This form includes several sections which your provider will go over with you.  Several symptoms are noted in each section, we ask that you circle anything that applies to you in the previous 6 months, otherwise just circle normal. 

For women this exam will include a breast exam and possibly a pap smear.  For men this exam may include a prostate exam.

Commercial Driving Exam also known as, CDL card or ODOT Physical.  For this exam you will need to fill out a Medical Examination Report  issued by DMV (we keep a supply of these on hand) and bring your driver’s license with you.  During this exam you’ll have your height and weight taken, have an eye exam and take a urine test.  Your provider will go over your current health history with you and then complete the exam.
