Living Well with Chronic Conditions

 

Sponsored by Samaritan Pacific Communities Hospital, Lincoln County Health and Human Services, and Oregon Department of Human Services.

 

A Self-Management Program.  Helping people who have significant health conditions learn how to take control of their health.

 

Developed by Stanford University’s School of Medicine and designed to enhance medical treatment, this program helps those living with a chronic health condition improve their ability to manage day-to-day activities.

 

This is a six session, weekly, interactive workshop.  Each session is 2.5 hours long.  Fee is $30.00, with scholarships available for those unable to pay.

&

February 1, 2007 in Lincoln City ~ 2:00—4:30 pm
